I have been running the Kodiac system for 15 to 20 K and no bearing problem. It may have been a bad bearing to start or if you have had other work done on that wheel, rotor pulled for any reason, possibly it was not reassembled correctly.
They are all individually assembled so I would not worry about the other 3.
